CMV-colitis in a pediatric non-transplant ALL patient receiving chemotherapy; A case report

Cytomegalovirus (CMV) is the leading cause of viral-associated congenital infections. Moreover, it can also be acquired. Between 50 to 80 percent world’s population seropositive for CMV and most clinical disease occurs in individuals previously infected with CMV. Rarely, serious infection has occurred healthy immune system. In contrast immunocompetent patients, higher morbidity mortality end organ considered immunocompromised patients. According available evidence, gastrointestinal (GI) lower prevalence case CMV-induced involvement, especially pediatric non-transplant acute leukemia. this report, we present a 12-year-old girl, known lymphoblastic leukemia (ALL) receiving maintenance chemotherapy manifestations gastroenteritis significant weight loss. Initial laboratory data, demonstrated mild pancytopenia lymphopenia thrombocytopenia. After excluding more common etiologies, colonoscopy multiple biopsies were taken which was indicative CMV-colitis. Intravenous (IV) ganciclovir 3 weeks oral valganciclovir about 9 months initiated. Follow-up courses surveillance included blood qualitative polymerase chain reaction (PCR) biopsy negative but tissue PCR positive 7 after initiation treatment. Oral treatment decided continued. To sum up, plenty guidelines have been developed stem cell transplantation human immunodeficiency virus (HIV) patients leukemic setting, neglected area field management.
